RN, Care Manager - Full Time, Home Health
Affinity Hospice is actively looking for a RN who is passionate about giving exceptional care to our hospice patients.

Primary functions are to coordinate patient care with the interdisciplinary team, including physician, patient/family and referring agency.
Administer skilled nursing care for all ages in the patients place of residence and assumes the responsibility for coordination of care.
Implement/develop/document the patient plan of care Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
Assure continuity of quality patient care delivered with appropriate documentation.
Promote agency philosophy and administrative policies.
Supervises and provides direction to the home health aide and the LPN.
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Current State license as a Registered Nurse, current Drivers License
Experience:
Two years of experience as a Registered Nurse in a clinical care setting, home health or hospice preferred.
Skills:
Nursing skills as defined as generally accepted standards of practice.
Proof of CPR, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption.
Recommended Skills Administration Cpr Certified Nurse Practitioner Clinical Works Health Care Home Care Estimated Salary: $20 to $28 per hour based on qualifications.
